Navigating the NDIS with Expertise and Compassion.
A Support Coordinator is someone who can find you the right supports, write a care plan of what you want and need, network with Allied Health and Service Providers, keep track of the budgets and spending and ensure you have support at your reviews and when things change. We will write Letters of Support to be sent to the NDIA, we will keep in regular contact with you, and organise reports to be done by your Allied Health Team whenever needed. Our role is to be there to act on your behalf.
There are three levels of support coordination:
Level 1 - Support Connection
Level 2 - Support Coordination
Level 3 - Specialised Support Coordination
